RESUMO - RACIONAL: Custos hospitalares em cirurgia constituem um peso para o sistema de saúde. Protocolos multimodais como o projeto ACERTO aceleram a recuperação pós-operatória. OBJETIVO: O objetivo deste estudo foi o de analisar custos hospitalares em pacientes submetidos a procedimentos cirúrgicos de grande porte no aparelho digestivo com ou sem as estratégias de cuidados perioperatórios proposta pelo projeto ACERTO. MÉTODOS: Foram coletados dados retrospectivos de pacientes eletivos submetidos a procedimentos cirúrgicos de grande porte no aparelho digestivo em um Hospital Universitário entre Janeiro de 2002 e Dezembro de 2011. O estudo envolveu duas fases: Entre Janeiro de 2002 a Dezembro 2005 envolvendo casos internados antes da implementação do protocolo ACERTO (período pré-ACERTO) e casos operados entre Janeiro de 2006 a Dezembro de 2011, após a implementação (período ACERTO). O desfecho primário foi a comparação de custos hospitalares entre os dois períodos. Como desfechos secundários, comparou-se o tempo de internação (LOS), complicações pós-operatórias, taxa de infecção de sitio cirúrgico (ISS) e a mortalidade. RESULTADOS: Foram analisados 381 pacientes (239 do período pré-ACERTO e 142 do período ACERTO) submetidos a procedimento cirúrgicos de grande porte no trato gastrointestinal. Pacientes operados dentro do protocolo ACERTO apresentaram mediana (IQR) mediana de tempo de internação três dias menor (p=0.001) quando comparados ao período pré-ACERTO (mediana (IQR): 10 (12) vs. 13 (12) dias). A mortalidade foi similar entre os dois períodos. Entretanto, o risco de complicações pós-operatórias foi 29% maior (RR: 1.29; IC95%: 1.11 - 1.50) no período pré-ACERTO (p=0.002). O risco de SSI também foi maior no período pré-ACERTO (RR: 1.33; 95%CI: 1.14-1.50). Custos (media e SE) per paciente foram de R$ 24562,84 (1349,33) antes da implementação e R$ 19912,81 (1459,89) após o protocolo ACERTO (p=0.02). CONCLUSÕES: A implementação do projeto ACERTO neste hospital universitário reduziu custos hospitalares em cirurgias digestivas de grande porte. Além disso, a prescrição de estratégias modernas de cuidados perioperatórios também reduziu riscos de complicações pós-operatórias e de SSI e o tempo de internação.

ABSTRACT - BACKGROUND: Hospital costs in surgery constitute a burden for the health system in all over the world. Multimodal protocols such as the ACERTO project enhance postoperative recovery. OBJECTIVE: The aim of this study was to analyze the hospital costs in patients undergoing major digestive surgical procedures with or without the perioperative care strategies proposed by the ACERTO project. METHODS: Retrospective data from elective patients undergoing major digestive surgical procedures in a university hospital between January 2002 and December 2011 were collected. The investigation involved two phases: between January 2002 and December 2005, covering cases admitted before the implementation of the ACERTO protocol (pre-ACERTO period), and cases operated between January 2006 and December 2011, after implementation (ACERTO period). The primary outcome was the comparison of hospital costs between the two periods. As secondary end point, we compared length of stay (LOS), postoperative complications, surgical-site infection (SSI) rate, and mortality. RESULTS: We analyzed 381 patients (239 of the pre-ACERTO period and 142 of the ACERTO period) who underwent major procedures on the gastrointestinal tract. Patients operated after within the ACERTO protocol postoperative LOS had a median of 3 days shorter (p=0.001) when compared with pre-ACERTO period [median (IQR): 10 (12) days vs. 13 (12) days]. Mortality was similar between the two periods. Postoperative complications risk, however, was 29% greater (RR: 1.29; 95%CI 1.11-1.50) in the pre-ACERTO period (p=0.002). SSI risk was also greater in pre-ACERTO period (RR: 1.33; 95%CI 1.14-1.50). Costs (mean and SE) per patients were R$24,562.84 (1,349.33) before the implementation and R$19,912.81 (1,459.89) after the ACERTO protocol (p=0.02). CONCLUSION: The implementation of the ACERTO project in this University Hospital reduced the hospital costs in major digestive procedures. Moreover, the implementation of this modern perioperative care strategy also reduced postoperative complications, SSI risks, and LOS.

RESUMO Objetivo: investigar em uma série de casos de pacientes submetidos a operações de médio porte em cirurgia geral, o uso da conduta de realimentação pós-operatória “ultra precoce”(dieta oral líquida oferecida na recuperação pós-anestésica), avaliando-se o volume de fluidos endovenosos recebidos no pós-operatório por estes pacientes, assim como a ocorrência de complicações e o tempo de internação hospitalar. Métodos: estudo prospectivo, observacional. Avaliou-se a aderência à rotina de realimentação “ultra precoce”, abreviação do jejum pré-operatório, volume de hidratação venosa perioperatório, tempo de internação e morbidade operatória. Resultados: um total de 154 pacientes com média da idade de 46±15 anos, foram acompanhados. Realimentação “ultra precoce” foi realizada em 144 casos (93,5%). Pacientes que não receberam realimentação “ultra precoce” receberam volume significativamente maior de fluidos endovenosos no pós-operatório do que pacientes realimentados de maneira “ultra precoce” (500ml versus 200ml, p=0,018). O tempo de internação foi de 2,4±2,79 dias (realimentação convencional) versus 1,45±1,83 dias (realimentação “ultra precoce”), sem diferença estatística (p=0,133).Não houve diferença no percentual de complicações gerais (p=0,291), vômitos (p=0,696) ou infecção do sítio cirúrgico (p=0,534). Conclusão: a realimentação “ultra precoce” apresentou-se como uma conduta de elevada aderência em operações de médio porte em Cirurgia Geral nesta série de casos e, esteve relacionada a infusão de volume significativamente menor de fluidos endovenosos no pós-operatório, com índices baixos de complicações e sem impacto no tempo de internação.

ABSTRACT Objective: To investigate the use of “ultra-early” postoperative feeding (oral liquid diet offered in the post-anesthetic recovery room) in patients undergoing common general surgical procedures and to assess the volume of intravenous fluids, as well as the rate of complications and the length of hospital stay. Methods: Prospective, observational study, which assessed the compliance with the “ultra-early” feeding, the reduction of preoperative fasting time, the perioperative venous hydration volume, the length of stay and the operative morbidity. Results: 154 patients with a mean age of 46 ± 15 years were followed. “Ultra-early” feeding was performed in 144 cases (93.5%). Patients who did not receive the “ultra-early” feeding received a significantly greater volume of postoperative intravenous fluids (500mL versus 200mL, p = 0.018). The length of stay was 2.4 ± 2.79 days (conventional feeding) versus 1.45 ± 1.83 days (“ultra-early” feeding), with no statistical difference (p = 0.133). There was no difference in the percentage of general complications (p = 0.291), vomiting (p = 0.696) or surgical infection (p = 0.534). Conclusion: “Ultra-early” feeding had a high adherence by patients undergoing common general surgical procedures, and it was related to decreased infusion of postoperative fluids. Complication rates and the length of stay were similar between groups.

RESUMO Racional: Protocolos multimodais de cuidados perioperatórios melhoram significativamente resultados na cirurgia. Objetivo: Investigar fatores de risco para vários desfechos clínicos em pacientes submetidos às operações colorretais eletivas com o emprego do protocolo ACERTO. Métodos: Coorte analisando indivíduos expostos ou não expostos às variáveis de risco demográficas, clínicas e intervenções ACERTO, através de um modelo de regressão logística, determinando fatores independentes de risco para morbidade e mortalidade. Resultados: Duzentos e trinta e quatro pacientes foram submetidos a 156 (66,7%) operações retais e 78 (33,3%) colônicas. Mantiveram relação com tempo de internação ≥7 dias operação retal e escore NNIS alto; jejum pré-operatório > 4h e volume de cristalóides >30 ml/kg/dia. O risco de infecção de sítio cirúrgico foi aproximadamente quatro vezes maior em desnutridos; oito em operações retais; e quatro com NNIS alto. Tempo de jejum pré-operatório ≤4 h reduziu em 81,3% o risco de infecção de sitio cirúrgico. Risco aumentado para fístula ocorreu em desnutridos, operação retal e escore NNIS elevado. Tempo de jejum pré-operatório ≤4 h constituiu fator de proteção para ocorrência de fístulas. Os fatores associados à pneumonia/atelectasia foram câncer e operação retal, enquanto que tempo de jejum pré-operatório ≤4 h e volume de cristalóides intravenoso ≤30 ml/kg/dia foram fatores de proteção. Mortalidade foi menor com jejum ≤4 h e fluidos endovenosos ≤30 ml/kg/dia. Conclusão: Este estudo permite concluir que operações retais, presença de fator de risco NNIS, tempo de jejum pré-operatório superior a 4 h e fluidoterapia com cristaloides endovenosos superior a 30 ml/kg/dia nas primeiras 48 h de pós-operatório constituem-se em fatores de risco independentes e aplicáveis para: 1) tempo de internação pós-operatória prolongada; 2) para infecção do sítio cirúrgico e fístula anastomótica associadas à desnutrição; 3) para pneumonia/atelectasia no pós-operatório; e 4) para mortalidade pós-operatória.

ABSTRACT Background: Perioperative care multimodal protocol significantly improve outcome in surgery. Aim: To investigate risk factors to various endpoints in patients submitted to elective colorectal operations under the ACERTO protocol. Methods: Cohort study analyzing through a logistic regression model able to assess independent risk factors for morbidity and mortality, patients submitted to elective open colon and/or rectum resection and primary anastomosis who were either exposed or non-exposed to demographic, clinical, and ACERTO interventions. Results: Two hundred thirty four patients were analyzed and submitted to 156 (66.7%) rectal and 78 (33.3%) colonic procedures. The length of hospital postoperative stay (LOS) ≥ 7 days was related to rectal surgery and high NNIS risk index; preoperative fasting ≤4 h (OR=0.250; CI95=0.114-0.551) and intravenous volume of crystalloid infused > 30ml/kg/day (OR=0.290; CI95=0.119-0.706). The risk of postoperative site infection (SSI) was approximately four times greater in malnourished; eight in rectal surgery and four in high NNIS index. The duration of preoperative fasting ≤4 h was a protective factor by reducing by 81.3% the risk of surgical site infection (SSI). An increased risk for anastomotic fistula was found in malnutrition, rectal surgery and high NNIS index. Conversely, preoperative fasting ≤4 h (OR=0.11; CI95=0.05-0.25; p<0.0001) decreased the risk of fistula. Factors associated with pneumonia-atelectasis were cancer and rectal surgery, while preoperative fasting ≤ 4 h (OR=0.10; CI95=0.04-0.24; p<0.0001) and intravenous crystalloid ≤ 30 ml/kg/day (OR=0.36; CI95=0.13-0.97, p=0.044) shown to decrease the risk. Mortality was lower with preoperative fasting ≤4 h and intravenous crystalloids infused ≤30 ml/kg/day. Conclusion: This study allows to conclude that rectal procedures, high NNIS index, preoperative fasting higher than 4 h and intravenous fluids greater than 30 ml/kg/day during the first 48 h after surgery are independent risk factors for: 1) prolonged LOS; 2) surgical site infection and anastomotic fistula associated with malnutrition; 3) postoperative pneumonia-atelectasis; and 4) postoperative mortality.

Um número significativo de mortes no trauma ocorre dias a semanas após a injúria inicial, sendo causado por infecções e insuficiência orgânica, relacionadas a hipercatabolismo e consequente desnutrição proteica aguda. A terapia nutricional deve ser planejada e incluída com as demais condutas de reanimação para pacientes politraumatizados e grandes queimados. A rápida aquisição de uma via para suporte nutricional é importante para inicio da terapia nutricional precoce em até 48 horas do atendimento. A via enteral é a opção preferencial no pós-operatório de pacientes traumatizado mas a via parenteral deve ser prescrita quando a enteral está contraindicada ou insuficiente. Após as medidas iniciais ditadas pelo ATLS, sintetizadas em A (air), B (breath), C (circulation), D (disability) e E (exposure), nós incluímos a letra F (feed) para enfatizar a importância do atendimento nutricional precoce no trauma.

A significant number of deaths in trauma occurs days to weeks after the initial injury, being caused by infection and organ failure related to hypercatabolism and consequent acute protein malnutrition. Nutritional therapy should be planned and included with other routines of resuscitation for patients with multiple trauma and severe burns. The rapid acquisition of a route for nutritional support is important to start early nutritional therapy within 48 hours of care. The enteral route is the preferred option in traumatized postoperative patients but the parenteral route should be prescribed when enteral feeding is contraindicated or inadequate. After the initial measures dictated by ATLS, synthesized in the A (airway), B (breathing), C (circulation), D (disability) and E (exposure), we include the letter F (feed) to emphasize the importance of early nutritional care in trauma.

OBJETIVO: Avaliar resultados pós-operatórios de pacientes do Serviço de Cirurgia Geral do Hospital Universitário Julio Muller antes e após a implantação do protocolo multimodal ACERTO. MÉTODO: Estudo retrospectivo em fichas preenchidas prospectivamente referentes a 5974 pacientes. Foram comparadas duas fases: de Janeiro de 2002 a Dezembro de 2004 (antes da implantação do protocolo ACERTO: período AA, n=1987); e de Janeiro de 2005 a Dezembro de 2008 (após a implantação do mesmo: período DA, n=3987). Comparou-se tempo de internação hospitalar, hemotransfusões realizadas, infecções de sítio cirúrgico (ISC), complicações operatórias e óbitos. RESULTADOS: Houve uma diminuição em um dia no tempo de internação entre o período AA e DA (mediana [variação]: 4 [0-137] vs. 3 [0-126] dias e moda: 3 vs. 2 dias; p<0,001). No período AA houve uma relação de 2,53 bolsas de hemoderivados transfundidas por paciente contra 0,77 no período DA (p<0,001). Notou-se tendência decrescente no número de casos de ISC ao longo dos anos estudados (A=-153,08; AA:7,51% vs. DA: 3,36% (p<0,001; RR=2,23; IC95%:1,73-2,89). Houve ainda tendência decrescente em relação a complicações operatórias (A=-51,41; AA:7,9% vs. DA: 6,14%; p=0,02; RR=1,29; IC95%:1,03-1,60), reoperações (A=-57,10; AA:/2,65% vs. DA:1,19%; p<0,001; RR=2,22; IC95% 1,43-3,44) e óbitos (A=-62,07;2,81% vs.1,73%; p<<0,01; RR=1,63; IC95%:1,15-2,31). CONCLUSÃO: A introdução do protocolo ACERTO proporcionou melhora dos resultados cirúrgicos, expressos por menor tempo de permanência hospitalar, uso de hemoderivados, e diminuição dos casos de infecção do sítio cirúrgico, complicações operatórias e óbitos.

OBJECTIVE:To evaluate the postoperative outcomes of patients in the Department of General Surgery, University Hospital Julio Muller, before and after implementation of the ACERTO multimodal protocol. METHODS: We conducted a retrospective study from 5974 patients' charts. We compared two periods: from January 2002 to December 2004 (before implementation of the ACERTO protocol: AA period, n = 1987) and January 2005 to December 2008 (after implementation of the protocol: DA period, n = 3987). The variables studied were length of hospital stay, blood transfusions, surgical site infections (SSI), postoperative complications and deaths. RESULTS: There was a decrease in one day in length of stay between the AA and DA period (median [range]: 4 [0-137] vs 3. [0-126] days and mode: 3 vs. 2 days, p < 0.001). During AA there was a relationship of 2.53 packs of blood products transfused per patient against 0.77 in the DA period (p <0.001). A downward trend in the number of cases of SSI was noticeable over the years (A =-153.08; AA: 7.51% vs. DA: 3.36% (p <0.001, RR = 2.23, 95 % CI:1.73-2.89). There was also a decreasing trend in operative complications (A =- 51.41, AA: 7.9% vs. DA: 6.14%, p = 0.02, RR = 1.29, 95% CI:1.03-1.60), reoperation (A =- 57.10; AA: 2.65%. vs DA: 1.19%, p <0.001, RR = 2, 22, 95% CI: 1.43 to 3.44) and deaths (A =- 62.07, 2.81 vs. 1%, 73%, p <0.01, RR = 1.63, 95% CI: 1.15 to 2.31). CONCLUSION: The introduction of the ACERTO protocol improved the surgical results, expressed as a shorter hospital stay, blood transfusion, and reduction in cases of surgical site infection, postoperative complications and deaths.

OBJETIVO: Avaliar os resultados pós-operatórios de um protocolo multimodal de cuidados peri-operatórios sem preparo mecânico de cólon (protocolo ACERTO) em pacientes submetidos a operações colorretais. METODOS: Foram avaliados prospectivamente 53 pacientes (37M e 16F; 57 [18-82] anos) submetidos à diversas operações colorretais com pelo menos uma anastomose divididos em dois grupos. O primeiro grupo (n=25) foi operado entre Janeiro de 2004 e Julho de 2005 com protocolo convencional incluindo preparo mecânico de cólon. O segundo grupo (n=28) foi operado entre agosto de 2005 e junho de 2008, após a implantação do protocolo ACERTO e sem preparo de cólon. Comparou-se estatisticamente a incidência de complicações, a duração da hospitalização e a mortalidade em ambos os grupos. RESULTADOS: Dois (3,8%) pacientes faleceram no pós-operatório, um em cada grupo. Pacientes do grupo ACERTO tiveram jejum pré-operatório abreviado, receberam menos fluido intravenoso e re-alimentaram mais cedo que o grupo convencional (p<0.05). Não houve diferença na morbidade pós-operatória (36% vs. 28,6%; p=0,56) com incidência de fístula anastomótica semelhante (12 vs. 10,7%; p=1,00). O número de complicações por paciente foi menor no grupo ACERTO (p=0.01). O tempo de internação do grupo ACERTO, operado sem preparo de cólon foi abreviado em 4,5 dias (12 [4-43] dias vs 7,5 [3-47] dias, p = 0,04). CONCLUSÃO: As rotinas do protocolo ACERTO são seguras e melhoram resultados em cirurgia colorretal por diminuir gravidade de complicações e o tempo de internação.

OBJECTIVE: To evaluate the outcomes after the implementation of a multimodal protocol (ACERTO protocol) with patients undergoing colorectal operations. METHODS: Fifty-three patients (37 M and 16 F; 57 [18-82] years old) submitted to various colorectal operations were prospectively studied in two different periods of time: from January 2004 through July 2005 (n=25, conventional group) and from August 2005 through June 2008 (n=28; ACERTO group). The patients received either the traditional perioperative management (including mechanical bowel cleansing) or a multidisciplinary protocol of perioperative care (without mechanical bowel cleansing) established by the ACERTO protocol. We looked at morbidity and mortality rates and length of hospital stay for comparisons between the groups. RESULTS: Mortality was 3.8% (2 patients) without difference between groups. Preoperative fasting and postoperative feeding were shortened and intravenous fluids were diminished in patients operated under the ACERTO protocol (p<0.05). Postoperative morbidity (36% vs. 28.6%; p=0.56) and the incidence of anastomotic leak (12 vs. 10.7%; p=1.00) were similar. The number of complications per patient with any complication was lower in the ACERTO group (p=0.01). Changing protocols reduced the length of hospital stay by 4.5 days (12 [4-43] vs. 7.5 [3-47] days, p= 0.04). CONCLUSION: The multidisciplinary routines of the ACERTO protocol are safe and enhanced recovery in colorectal surgery by reducing both hospitalization and the severity of postoperative morbidity.

OBJETIVO: Investigar se a adoção de um protocolo de restrição hídrica intravenosa em colecistectomia aberta diminui a permanência hospitalar. MÉTODO: Estudo prospectivo envolvendo pacientes submetidas a colecistectomia aberta após a adoção de um protocolo multimodal (projeto ACERTO). Pacientes com intercorrencias intra- ou pós-operatórias foram excluídas. Todas as pacientes receberam uma solução de carboidratos 2h antes da operação. Foram coletadas as seguintes variáveis: peso, altura, estado nutricional, volume total de fluidos intravenosos (VTFI) e volume prescrito em ml/kg de peso por dia. RESULTADOS: 64 pacientes (11 M e 53 F; idade mediana = 43 [18-65] anos) completaram o estudo. A re-alimentação pós-operatória ocorreu no mesmo dia da operação (62,5%) ou no dia seguinte (37,5%). Não foram registradas complicações infecciosas ou óbitos. O tempo mediano de internação pós-operatória foi de 1 (1-4) dia. Observou-se correlação entre o VTFI (r=0,44; p<0.001) e volume de fluídos / kg peso /dia (r=-0,29; p=0,03) e o tempo de internação pós-operatória. Alta no 1º PO foi possível em 73,3% dos casos quando a prescrição foi de até 20mL/Kg/dia e em 41,2% quando o volume prescrito foi maior (p<0.001; Odds Ratio=3,92; IC95% 1,36-11,32). CONCLUSÃO: A restrição de fluidos intravenosos em colecistectomia aberta determina alta mais precoce.

BACKGROUND: The aim of this stud y was to investigate the adoption of a protocol of intravenous fluid restriction in the length of hospital stay after open cholecystectomy. METHOD: Prospective study including female patients undergoing open cholecystectomy after the adoption of a new protocol (ACERTO protocol). Patients with intra- or postoperative interferences with original operation or normal course were excluded. All patients receive a carbohydrate beverage 2h before operation. Variables collected were: weight, height, nutritional status, total volume of intravenous fluids (VTFI) and volume received in ml/kg of weight/day. RESULTS: 64 patients (11 M and 53 F; median age = 43 [18-65] years) entered the study. Postoperative re-feeding occurred in the same day of the operation (62.5%) or in the next day (37.5%). There were no deaths or postoperative infectious complications. Median length of postoperative stay was 1 (1-4) days. Discharge on the 1st PO day was possible in 73.3% when prescription was at least 20 mL/kg/day and in 41.2% when this volume was greater ((p<0.001; Odds Ratio=3,92; IC95% 1,36-11,32). CONCLUSION: Restriction of postoperative intravenous fluids leads to early hospital discharge after open cholecystectomy.

RACIONAL: O índice de massa corporal vem sendo usado indistintamente entre idosos ou não para a avaliação do estado nutricional. A hipótese deste trabalho é a de que em pacientes idosos um ponto de corte mais alto para o índice de massa corporal pode ter maior validade na aferição do risco nutricional em cirurgia. OBJETIVO: Avaliar a morbimortalidade em pacientes cirúrgicos idosos comparada a pacientes mais jovens e sua associação com valores baixos de índice de massa corporal utilizando-se um ponto de corte convencional (18,5 kg/m²) e outro adaptado para pacientes com idade avançada (24 kg/m²). PACIENTES E MÉTODOS: Um mil e novecentos e doze pacientes foram divididos em dois grupos: <65 anos n = 1627) e >65 anos (n = 285). Comparou-se nos dois grupos o tempo de internação e a morbimortalidade. Pacientes com idade >65 anos foram estratificados segundo o índice de massa corporal <18,5 kg/m² ou <24 kg/m², e analisados quanto a complicações pós-operatórias, reoperações, óbito e tempo de internação. RESULTADOS: O tempo de internação foi maior em pacientes com idade >65 anos (6 [1-75] dias vs. 4 [1-137] dias), assim como as complicações pós-operatórias (37/285; 13,0% vs. 109/1627; 6,7%; OR 2,1; IC95% 1,40-3,09) e óbitos (15/285; 5,3% vs. 34/1627; 2,1%; OR 2,6; IC95% 1,40-4,84). Em pacientes com índice de massa corporal < 24 kg/m², o tempo de internação (8 [1-75] dias vs. 4 [1-43] dias), complicações pós-operatórias (28/152; 18,4% vs. 9/133; 6,8%; OR 3,1; IC95% 1,41-6,86) e reoperações (16/152; 10,5% vs. 3/133; 2,2%; OR 5,1; IC95% 1,45-17,91) mostraram-se maiores. Utilizando-se como ponto de corte o índice de massa corporal de 18,5 kg/m², não houve diferenças. CONCLUSÕES: A morbimortalidade é alta em pacientes cirúrgicos de idade avançada. Nestes, o índice de massa corporal abaixo de 24 kg/m² mostrou melhor associação com a ocorrência de complicações pós-operatórias, reoperações e tempo prolongado de internação.

BACKGROUND: Standard body mass index cut-off points for malnutrition are routinely used for adults independently of their age. The hypothesis of this study was that a cut-off point higher than the usual for the diagnosis of malnutrition might be more precise to access the nutritional risk of aged surgical patients. AIM: To evaluate the morbimortality in aged surgical patients and its association with nutritional status assessed by body mass index using either the standard and a higher cut-off point for malnutrition. PATIENTS AND METHODS: All patients admitted for operative procedures (n = 1,912) were allocated to either two groups: <65 years (n = 1,627) or >65 years old (n = 285). The body mass index was used to access the nutritional status. Two different cut-off points for malnutrition (18.5 or 24 kg/m²) were used in the group of older patients. Endpoints of the study were length of stay, morbidity and mortality. RESULTS: Length of stay was higher in patients over 65 years (6 [1-75] days vs. 4 [1-137] days). Both postoperative complications (37/285; 13.0% vs. 109/1627; 6,7%; OR 2,1; IC95% 1,40-3,09) and deaths (15/285; 5.3% vs. 34/1627; 2,1%; OR 2,6; IC95% 1,40-4,84) were most common in the older group. Using a higher cut-off (24 kg/m²), the length of stay (8 [1-75] days vs. 4 [1-43] days), postoperative complications (28/152; 18.4% versus 9/133; 6.8%; OR 3,1; IC95% 1.41-6.86) and re-operations (16/152; 10.5% versus 3/133; 2.2%; OR 5.1; IC95% 1.45-17.91) were greater in malnourished patients. However these correlations were not statistically significant with the cut-off point for malnutrition in 18.5 kg/m². CONCLUSIONS: Morbimortality is higher in surgical patients over 65 years old. In these, the cut-off point set in 24 kg/m² was most associated with the occurrence of postoperative complications, re-operations and length of stay.

OBJETIVO: O objetivo do presente estudo foi avaliar os resultados clínicos iniciais após a implementação do projeto Acerto Pós-operatório (ACERTO) em pacientes operados num serviço universitário de cirurgia geral. MÉTODO: 161 pacientes foram prospectivamente observados durante dois períodos: o primeiro, entre janeiro a junho de 2005 (n=77) formado por pacientes submetidos a condutas convencionais (período PRÉ-ACERTO) e o segundo, entre agosto a dezembro de 2005 (n=84), formado por pacientes submetidos a um novo protocolo de condutas peri-operatório estabelecidas pelo projeto ACERTO (período PÓS-ACERTO). A coleta de dados nos dois períodos ocorreu sem o conhecimento dos profissionais do serviço. As variáveis observadas foram: indicação de suporte nutricional pré-operatório, tempo de jejum pré e pós operatório, volume de hidratação, uso de sondas e drenos, tempo de internação e morbidade pós-operatória. RESULTADOS: Na fase pós-ACERTO, o percentual de pacientes desnutridos que receberam suporte nutricional foi trêsês vezes maior (78,6%; 11/14 vs. 23,5%; 4/17; p <0.01). Houve uma diminuição no tempo de jejum pré (16 [8-27] vs 5 [2-20] horas, p<0,01) e pós-operatório (3 [1-15] vs 1 [1-6] dias, p<0,01) e na hidratação venosa (8 [1-63,5] vs 4 [0,5-63] litros, p<0,001) na segunda fase do estudo. O conjunto de mudanças determinou uma redução de dois dias no tempo de internação (5 [2-46] vs 3 [1-64] dias, p<0,05) e de 60% na morbidade pós-operatória (18,2%;14/77 vs 7,1%;6/84, p=0,03; RR=2,55, IC95%1,03-6,29, p<0,05). CONCLUSÕES: A adoção das medidas multidisciplinares peri-operatórias como as do projeto ACERTO é factível dentro da nossa realidade e pode, melhorar a morbidade e diminuir o tempo de internação em cirurgia geral.

BACKGROUND: The aim of this study was to evaluate the initial results after the implementation of the ACERTO project in patients submitted to abdominal operations at the Julio Muller Universitary Hospital. METHODS: 161 consecutive patients were studied in two different periods: from January to July 2005 and from August to December 2005 (n=84; POST-ACERTO phase). The patients received either traditional peri-operative management (n=77; PRE-ACERTO phase) or a multidisciplinary protocol of peri-operative care (n=84; POST-ACERTO phase) established by the ACERTO project. During both periods, the surgical staff was unaware of the prospective data collection. RESULTS: The number of malnourished patients receiving pre-operative nutritional support was higher after the implementation of the project (78.6%; 11/14 vs. 23.5%; 4/17; p <0.01).This second phase was associated with a decrease in both pre (16 [8-27] vs 5 [2-20] hours, p<0,01) and post-operative fasting (3 [1-15] vs 1 [1-6] days, p<0.01), and in the volume of intravenous fluids (8 [1-63.5] vs 4 [0.5-63] L, p<0.001). The changing of protocols reduced the hospital length of stay by two days (5 [2-46] vs 3 [1-64] days, p<0,05) and the postoperative morbidity by 60% (18.2%;14/77 vs 7.1%;6/84, p=0.03; RR=2,55, IC95%1.03-6.29, p<0.05). CONCLUSION: The implantation of a multidisciplinary protocol such as those contained in the ACERTO project is feasible in this country and may decrease both hospitalization and postoperative morbidity after general surgery.
